Tau protein is an intracellular, microtubule-associated axonal phosphoprotein and is involved in the regulation of microtubule dynamic stability. Among brain tissue components, axons appear to be most susceptible to injury after TBI. Tau lesions are related to axonal disruption and are being investigated as a potential biomarker of central nervous system (CNS) injury . Presently, plasma P-Tau (phosphorylated-Tau), total Tau (T-Tau) and the ratio of P-Tau:T-Tau are considered as promising diagnostic and prognostic biomarkers for acute and chronic TBI . Moreover, P-Tau concentration and P-Tau:T-Tau ratio significantly correlated with CT findings, and P-Tau concentration consistently showed the highest diagnostic accuracy across all age-groups.
Tau phosphorylation plays an important role in brain physiology and pathology. During foetal development, it supports microtubule dynamics and neuroplasticity, whereas in Alzheimer’s disease (AD), it drives pathological tau aggregation and tangle formation.
